The required release forms for any adult, volunteer, or child who wishes to participate in a diocesan event in the Diocese of Fort Worth.

Form A - (one per child) needs to be filled out once a year. The form is effective June 1 - May 31
Form B - (one per child) needs to be filled out for the specific event you are attending.
Form D - (one per adult) need needs to be filled out once a year. The form is effective June 1 - May 31
